In order to participate in interscholastic competition, athletic participants must have on file in the Athletic Office:

    1. Athletic Physical Card (yellow) which includes Consent for Emergency Care
        and Record of Insurance/Parent Permission Sections.

    2. Warning Film Card (blue) (available from the coach after viewing the video)

    3. Birth Certificate
